MyPHR Patient Portal
At AVC Primary Care, you can securely access your medical records online through the MyPHR patient portal, which is powered by CharmHealth.
During the sign-up process, you will be invited to create your CharmHealth portal account. CharmHealth will send you an email with instructions to activate your account and establish your login credentials.
How to Access Your Patient Portal
1. Check your email for the CharmHealth invitation email.
2. Follow the instructions to activate your account and set up your password.
3. Log in to the patient portal at: https://phr.charmtracker.com
Mobile Access
You can also access your health records through the Charm mPHR mobile app, which functions similarly to MyChart, allowing you to view your health information from your phone.
The app allows you to:
- View medical records and visit summaries
- Access lab results and health documents
- Review treatment plans and provider notes
- Manage and update your personal health information
